Requirements for Admission to the First Year All applicants for admission to the medical college are screened by the Admission Committee. After a thorough study of all pertinent information, the Admission Committee makes a list of those who in their opinion are best suited for the study of the medicine and recommends that they be accepted for enrolment to the first year level. There are several requirements that have to be fulfilled before any applicant can be considered by the Admission Committee.
2. The applicant must fill out completely and accurately one (1) application form to be submitted to the Admission Office. 3. The applicant must submit his/her complete transcript of records of all college grades obtained. If the applicant is an undergraduate, he should submit a complete transcript of all his/her grades up to the first semester prior to his graduation and should include a certificate from the Registrar of his/her institution stating the subjects he/she is taking in the second semester of the school year immediately preceding application. He/she should list down all other requirements and the school he/she intends to complete them including summer sessions. To be eligible for admission, he/she should pass all these courses. 4. The applicant must also submit the result of his National Medical Admission Test (NMAT). 5. Three (3) signed passport size pictures are required of each applicant to be submitted with the application form. The pictures should have been taken within sixty (60) days of the date of admission. 6. All applicants residing in the Philippines are required to pay an application and entrance exam fee.
Foreign Applicants All applicants for admission to the medical college are screened by the Admission Committee. All applicants not residing in the Philippines are required to pay an application fee of $ 200.
Requirements for Transferees 1. Honourable dismissal/Certificate of Transfer Credential. 2. Scholastic Record/Copy of Grades 3. NMAT Result (Score) 4. Certificate of Good Moral Character/Recommendation Letter from former College Professor, Dean and Registrar 5. Birth Certificate 6. Application Form 7. Two (2) Pieces 2x2 Picture 8. Five (5) Pieces Mailing Stamps 9. Validating Exam 10. Certificate of Eligibility for Admission (CEA) 11. One (1) Long Folder 12. Two (2) Long Brown Envelopes 13. One (1) long Plastic Envelope 1. Transcript of Record (TOR) 3. Finally, the applicant should pay to the Accounting Office the required Tuition fee per semester plus the miscellaneous fees
(For Irregular Students) 1. Secure clearance form from the Dean’s Office for Administrative and Academic clearance. 2. If approved, get academic clearance from the same office. 3. Proceed to Registrar’s Office for CHED and school requirements clearance. 4. Go back to Dean’s Office for Registration form. 5. Proceed to Accounting Office for student accounts, clearance, assessment and payment of tuition fees. 6. Fill up the form and submit each form to the corresponding department indicated therein. (For Regular Students)
